Medication Forms & Action Plans

According to the NJDOE, the following guidelines apply when administering medications:

  1. Every school year, the parent/guardian must present a written and signed physician order along with written parental permission. The order must include:
    1. An outline of the pupil's diagnosis.
    2. The name, dosage, and time of administration along with potential side effects.
    3. Any restrictions on the student's activities (e.g., lab, PE, or shop classes).
    4. If medication is prescribed on an as-needed basis, the condition under which it is to be used.
    5. Other medications the student receives that might affect the ordered medication.
  2. All medications must be kept in the original container and administered only by the school nurse. Herbal remedies are not included. Medications must be brought to the Nurse's Office by the parent or guardian.
  3. Parents must notify the school nurse every school year if their child is carrying emergency medication such as EpiPens or inhalers.
  4. Any unused medication must be picked up by the parent/guardian on the last day of school, or it will be discarded.

AEDs – Each school building has an available automated external defibrillator (AED) that is unlocked and accessible during the school day for sudden cardiac events.

Epinephrine – Each school building has employees who volunteer to be trained and designated by the school nurse to administer epinephrine during an anaphylactic emergency.

General Information

Height, Weight, Vision, Hearing & Blood Pressure
Any parent/guardian, counselor, teacher, or student may request a vision or hearing screening at any time. Height, weight, and blood pressure screenings are conducted annually. Visual acuity is conducted biennially through grade 10 and auditory acuity in grades 7 and 11.

Scoliosis
A scoliosis examination is performed bi-annually on students between ages 10–18. Parents wishing to exclude their child must contact the school nurse.

Physical Exam
A medical exam is recommended at least once during each developmental stage (grades 7–12). Please contact your child's medical home and send any exam forms to the health office.

According to New Jersey law, every student is required to have the following immunizations:

  • DPT – 3 doses of Td or combination of DTP, DTaP, DT equal to 3 doses.
  • Tdap – 1 dose for students born on/after 1-1-97.
  • Polio – At least three doses of oral polio vaccine.
  • Measles – Two vaccinations required if born after 1/1/90, or laboratory evidence of immunity.
  • Mumps & Rubella – One vaccination or laboratory evidence of immunity.
  • Hepatitis B – Completed series or laboratory evidence of immunity.
  • Meningococcal – 1 dose for students born on/after 1-1-97.
  • Varicella – 1 dose required for children born on or after 1-1-98.
  • TB – Students transferring from a high TB incidence country may require a tuberculin test.
